If you arrive late for your appointment, we may need to reschedule your appointment.
You will need to bring the following:
- INSURANCE INFORMATION
- DRIVER'S LICENSE OR PICTURE IDENTIFICATION
Bring the following if you have:
- X-RAYS
- CT SCANS
- MRI SCANS
- ATTORNEY INFORMATION
If you have difficulty obtaining these records, please call us and we will assist you.
Without records, it may be necessary to reschedule your appointment.
